Opportunities missed prior to murder of child

Authorities missed opportunities to intervene before a teenager was murdered, an investigation has found.

A serious case review was conducted following the death of 13-year-old Lucy McHugh, who was raped and fatally stabbed by lodger Stephen Nicholson.
