Over the past six months we've seen a series of national plans to improve people's health and care services.

In its latest White Paper, Joining up care for people, places and populations, the Government sets out plans for better integration of the health and care system in England – an overarching aim that is broadly welcome and one which many in the NHS and local authorities are already working towards.
