When I worked in a humanitarian organisation, the best solutions often emerged from frontline practitioners improvising during a crisis - finding workarounds to seemingly impossible constraints. Necessity is the mother of innovation. 

I mention this because discussions about innovation too often start from scratch, ignoring the lessons we can already learn from the creativity you see in public services.
