A new report has warned councils and health trusts must work together to prevent homeless people being discharged from hospital.

Councils and health trusts must work together to prevent homeless people being discharged from hospital and ending up back on the streets, a new report has warned.

The study, published on 28 May by charities Homeless Link and St Mungo's, claimed more than 70% of homeless people were being discharged back on to streets.
