Medway's success in reducing delayed transfers of care

Chris McKenzie outlines how innovation and strong partnership working has enabled Medway to reduce levels of delayed transfers of care (DToC) to among some of the lowest levels in the region.

A strong partnership approach and investment in a range of impactful and innovative services has helped Medway to achieve and sustain some of the lowest rates of delayed transfers of care in the south east of England.

The average number of Delayed Transfers of Care (DToC) in Medway, during 2018/19, was 6.5 days per 100,000 population, of which 1.6 days were directly attributable to Medway Council. This compared with a national average of 14.2 DToC days per 100,000 population, with 3.1 days attributable to Councils.
