'Missed opportunities' in baby's death

Newcastle City Council has admitted there were ‘missed opportunities’ in the wake of a review into the death of a 16-week old child.

Newcastle City Council has admitted there were ‘missed opportunities' in the wake of a review into the death of a 16-week old child.

The serious case review by the Newcastle Children's Safeguarding Board into the death of Tequiilah Burke in December 2015 made 11 recommendations to help prevent similar tragedies in the future.
