The NHS needs a fundamental shift towards a less defensive and more open culture, according to the Parliamentary Health Committee's inquiry into complaints and litigation.

Key recommendations include a complete overhaul of the role of the Health Service Ombudsman to maintain an effective appeals process for the complaints system. It also urges a widening of the legal and operational framework guiding the Ombudsman, so that any complaint can be independently reviewed if rejected by a service provider.
