Ofsted's chief has warned of ‘blind spots' in the children's services inspection system that could mean youngsters are ‘potentially coming to significant harm'.

In her annual report, Amanda Spielman said the ‘overall story' was one of improvement for children's services, with those rated good or outstanding rising from 36% in 2013 to 48% while those rated inadequate have dropped from 22% to 12%.
