Supporting our homes, people, and communities this winter

By offering to develop streamlined hospital discharge pathways with commissioners, HC-One is improving outcomes for those admitted to its homes, says Jake Rollins.

HC-One is proud of the critical role we play every year in supporting the NHS and wider social care services during the challenging winter months. With over 300 homes across England, Scotland and Wales, this support spans hundreds of communities and local health and social care systems.

We are under no illusions that the shadow of the pandemic means this winter will likely add even more strain on the health and social care system. The continued effects of COVID-19, combined with the increased numbers of cold-related illnesses and accidents, and the fear there will be lower levels of flu resistance following last winter's social distancing and lockdowns, means everyone expects there to be an extremely high demand for care across the system.
