A major overhaul of care services is needed to tackle the causes of poor and undignified older people's care in England's homes and hospitals, according to a report by the Local Government Association, the NHS Confederation and Age UK.

Published on 18 June, the report is the culmination of long-running review by the three organisations' joint commission on care, which involved in-depth discussions with some 230 health and social care organisations.
