Recent headlines have stated that one in seven beds in acute hospitals are being occupied by patients who would be better off receiving care elsewhere.
This is sometimes a residential social care setting, but often it’s in their own home. Sadly, persisting issues around discharge processes continue to make it difficult for patients to be discharged - and into the right setting. Having been a social worker for more than two decades, I can safely say these issues aren’t new.
